Insurance information
Self-pay and the following insurance companies are accepted:
- Health Alliance (PPO)
- Aetna
- Blue Cross Blue Sheild (HMO/PPO)
- Healthlink
- Live 360
- Physicians Health Association of Illinois
- Cigna
- Optum/ United Behavioral Health/United Healthcare/UMR
Athenahealth electronic health record and practice management system is utilized. The service automatically checks your eligibility three days prior to your first appointment and submits all claims thereafter. However, the patient is also encouraged to contact their insurance carrier to ensure coverage.
At times, insurance providers require a referral or preauthorization for an evaluation or therapy. In such cases, the patient is responsible for checking and obtaining this.
Coverage cannot be guaranteed.
Cancellation Policy:
If you do not show for a scheduled appointment and have not contacted the office at least 24 hours in advance, you will be charged the full cost of the session.
